Matt Zaborowski is the Executive Director of the Association of State and Territorial Dental Directors (ASTDD) and President/CEO of the ASTDD Foundation, Inc. In these roles, he provides leadership, governance, and philanthropic strategy to advance, strengthen, and support state and territorial oral health programs. With broad experience across dental operations, population health and prevention programs, Medicaid reform, and state and national collaboration, Matt Zaborowski has spent the past decade partnering with policymakers, public health professionals, nonprofits, and oral health champions to expand access to oral health services and reduce disparities.

Dr. Adam Barefoot is a population-based oral health leader and Chief Dental Officer at Mission Mobile Medical, where he strives to improve access to care and improve oral health outcomes. His prior service as Georgia’s State Oral Health Director and Chief Dental Officer at HRSA reflects his deep commitment to strengthening state oral health programs, expanding access to prevention, and advancing data-driven solutions to improve oral health equity nationwide.
Adam has been engaged with ASTDD for many years, both directly and through his role as state dental director, and is a strong champion for state oral health programs and their national, state, and local partners. He is honored to help launch the ASTDD Foundation and is committed to building a robust landscape for community-based prevention, data and surveillance, and systems that ensure every child and family can benefit from evidence-based oral health care.
Zachary Brian, DMD, MHA
Director, 2026 – 2027
Dr. Zachary Brian is an oral health policy and systems leader and associate professor at the University of North Carolina at Chapel Hill Adams School of Dentistry, where he directs the Dentistry in Service to Community (DISC) program and founded the UNC Program for Oral Health Policy, reflecting his deep commitment to advancing health equity for underrepresented and marginalized communities through public health, education, and policy.
Zach has been an active partner to ASTDD and state oral health programs through his work on best practices, policy dissemination, and state-based initiatives, and he is honored to serve on the ASTDD Foundation Board, bringing his experience in funder relations, strategic planning, and multi-sector collaboration to help strengthen community-based prevention, data and surveillance, and systems that ensure every child and family can benefit from evidence-based oral health care.
Shelly Guinn, RDH, MPH
Secretary, 2026 – 2028
Ms. Shelley Guinn serves as Secretary for both the ASTDD and ASTDD Foundation Boards and as the State Oral Health Program Coordinator at the Washington State Department of Health. In her state role, she provides technical assistance, promotes school-based prevention strategies such as dental sealants and fluoride varnish, and partners with organizations, coalitions, clinics, and schools to expand access to oral health services across Washington. With more than two decades of experience spanning clinical dental hygiene, Head Start program coordination, nonprofit leadership, and state public health, she brings deep expertise in program development, budgeting, grant management, and community partnership building.
Nate Hornsby, JD, LLM
Director, 2026 – 2028
The Honorable Nashon Hornsby is a seasoned New Jersey public health leader who currently serves as Deputy Commissioner of the Population Health Branch (NJ Department of Health), drawing on more than two decades of state service in health policy, family health, health promotion, and oral health, including leadership of Oral Health Services and community health initiatives that expand access to prevention and care for children, families, and underserved communities.
Nashon has been a long-standing partner to ASTDD and state oral health leaders and is honored to support the launch of the ASTDD Foundation, bringing his commitment to health equity, cross-sector collaboration, and system-level solutions that strengthen oral health for all New Jerseyans and communities nationwide.
Rachel Sizemore, DDS, MHSA
Treasurer, 2026 – 2027
Dr. Rachel Sizemore serves as Treasurer of both the ASTDD and ASTDD Foundation Boards and as the State Dental Director for the Arkansas Department of Health’s Office of Oral Health. In her state leadership role, she oversees statewide oral health surveillance, prevention initiatives, and community partnerships focused on reducing disparities and improving access to evidence-based care. Dr. Sizemore brings frontline clinical experience, program management expertise, and a strong commitment to data-driven decision-making to her national service, helping guide ASTDD’s strategic priorities, fiscal stewardship, and systems-focused efforts to advance oral health.
Kevin Thomas
Director, 2026 – 2029
Kevin Thomas is a lifelong dental industry leader and co-president of Elevate Oral Care whose board service with organizations such as the Children’s Dental Health Project, Oral Health American, Dental Trade Alliance Foundation, and the American Academy of Pediatric Dentistry Foundation reflects his long-standing commitment to expanding access to preventive oral health care, supporting public health innovation, and tackling complex challenges facing oral health programs nationwide.
